Third Annual Madisyn Kemp Memorial
Golf Tournament

Plans are underway for the Third Annual Madisyn Kemp Memorial Golf Tournament on Friday,
Sept 26, 2008. This year's tournament will be held at the Meadows Golf Club located off
Hwy 280 east of Chelsea. Proceeds from this tournament will benefit SMA research and
families like Madisyn's who are directly affected by SMA.
Children with SMA are born with little muscle tone that continues to deteriorate over time
resulting in the loss of all voluntary muscle movement. These children cannot crawl,
walk, or even sit up. SMA affects their ability to swallow and breath and leads to
respiratory failure. SMA kills more babies than any other genetic disease.
After 3 ½ years, Madisyn lost her courageous battle with this disease on 3/21/2008. Her
parents, Brandon and Patti remain committed to continue the fight in her memory.
Registration will begin at 11:30 a.m. with lunch served from 11:30 until 12:45 p.m.
Tournament play will begin with a shotgun start at 1 p.m. All entries include green fees,
golf cart, lunch and beverages, and a gift bag.
This year we will also be offering three levels of corporate sponsorship.
Eagle Package: $ 600.00 includes four golfers, mulligans, and a tee box sign.
Birdie Package: $400.00 includes two golfers, mulligans, and a tee box sign.
Par Package: $300.00 includes one golfer, mulligans, and a tee box sign.
Foursome: $400.00.
Single: $100.00
Tee box sign only: $250.00
Mulligans (maximum of 5 per foursome): $5.00 each or 5 for $20.00.
Early registration deadline is August 29st. All golfers who are registered and have paid
by August 29st will receive a free T-shirt.
